ABSTRACT

Objective@#To understand the current situation of blood-borne occupational exposure among health care workers and evaluate the intervention effect of PDCA.@*Methods@#Retrospective survey was adopted to investigate and analyze the blood-borne occupational exposure incidents in a hospital from January 2015 to December 2018, and to compare the intervention effects after PDCA management.@*Results@#A total of 82 cases of occupational exposure occurred from 2015 to 2017, and only 9 cases happened after the implementation of PDCA intervention. The exposed population was mainly consisted of nurses (59 cases, 64.83%) , and mainly with low-working age (1-5 years) (56 cases, 61.54%) , and the main source of exposure was hepatitis B (34 cases, 37.36%) . In addition, after the implementation of PDCA, the vaccination rate of personnel was 77.78%, the standardized field treatment rate was 100%, the preventive drug use rate was 88.89%; The qualified rate of occupational protection assessment was higher than that before intervention, which all shows the difference was statistically significant (P<0.05) .@*Conclusion@#With the guide of PDCA management, Strengthen the training of new employees or ones with low working years as well as their awareness of protection, and standardize the relevant operational procedures, which can significantly improve the prevention of blood-borne occupational exposure and stress management of medical staff.

ABSTRACT

Objective To analyze the current situation of the quality of critical value management in ultrasound department,and to explore the application and effect of PDCA cycle method in critical value management.Methods The quality status of emergency data during the period from January 2015 to January 2019 after the PDCA cycle was adopted was analyzed.The PDCA cycle method was applied to improve the overall quality.The reporting rate,awareness rate,qualified rate of writing,and timely rate of clinical treatment were compared before and after the PDCA cycle was adopted to evaluate the effect of PDCA cycle method.Results From January 2015 to January 2019,the reporting rate was 96.4%,98.0%,94.5%,and 96.7%,the awareness rate of critical value was 63.2%,78.6%,84.8%,and 92.2%,the qualified rate of writing was 70.4%,77.2%,85.3%,and 94.0%,and the timely rate of clinical treatment was 50.9%,71.7%,81.1%,and 85.5%,respectively.The awareness rate,the qualified rate of writing,and the timely rate of clinical treatment were significantly improved (x2=12.464,38.184,52.926,all P < 0.05).Conclusion PDCA cycle method can improve the quality of emergency management in ultrasound department.

ABSTRACT

Objective To study the application effect of PDCA management mode in the teaching process of practice nurses in spine surgery to reduce the occurrence of nursing errors. Methods A total of 35 practice nurses in spine surgery from March 2014 to March 2015 received the routine teaching mode were set as control group, and 35 practice nurses from April 2015 to April 2016 implemented PDCA management mode were set as intervention group. The occurrence rate of nursing adverse events in two years was observed, and the interns for clinical teaching evaluation model of quality and satisfaction and the intern examination results were summarized. Results The incidence of nursing adverse events was 22.86% (8/35) in control group, and 2.86% (1/35) in intervention group, the difference was statistically significant (χ2= 17.85, P = 0.00). The intern teacher for clinical teaching content arrangement, teaching attitude, detail guidance and skills training indicators evaluation score were (15.46±1.03), (14.18±1.06), (14.61±1.11), (13.30±1.07) points in control group, and (18.26±1.00), (18.48±0.51), (19.43±0.21), (19.16± 0.28) points in intervention group, the differences were statistically significant (t = 11.54- 31.34, P <0.01). The interns given theory and skills examination results were (80.15±1.61), (85.16±1.02) points in control group, and (93.10±2.01), (91.06±2.03) points in intervention group, the difference was statistically significant (t = 29.75, 15.36, all P = 0.00). The clinical teaching satisfaction was 57.14% (20/35) in control group, and 80.00% (28/35) in intervention group, the difference was statistically significant (χ2=12.12, P=0.00). Conclusions PDCA management model applied to clinical practice in department of orthopedics teaching process, can effectively improve the intern theory knowledge and skills to master the ability, most of the interns examination results are more optimistic, and for this kind of management pattern for the process of teaching evaluation of higher satisfaction, the teachers care under the guidance of intern nursing mistake, nursing adverse events is low, this method is worthy of promotion.

Objective To study the effect of the PDCA management on safety management of venous transfusion by indwelling needles?.Method The measures included standardizing nurses manipulation,improving the management system related to venous transfusion,and regulating details management process for tube fixing and dressing and tube maintenance.Results The rates of unstandardized maintenance and obstruction of tubes after the enforcement of details management were significantly lower than those before its enforcement(P<0?05).Conclusions PDCA management is effective for standardizing nursing implementation and ensuring the safety of venous transfusion by indwelling needle?
